BEFORE REGISTERING

Joining as a new member or renewing HTAi membership is possible on the HTAi website.
Please be informed that the HTAi member’s 『Unique Identifier code (UID)』,which is sent by e-mail after confirming your membership registration, must be entered when registering for the HTAi 2013 Annual Meeting. If you do not want to become a member, you are allowed to register only for One-day and/or Workshops.

HOW TO REGISTER

- Go to the HTAi website to sign up for HTA 2013-2014 membership

- Sign up: Create your account on the HTAi2013 website.
- Sign-in: Use your E-mail and Password.

- Choose your registration fee type and optional events such as Workshops and Gala Dinner.

- Paying with a credit card using VISA, Master, and JCB or bank transfer is acceptable.
- Only registrations with appropriate payments can be honored.

If your payment has been completed, confirmation of your registration will be sent within two weeks via e-mail.

- Modification or updates can be made after signing in with your E-mail and Password until 31 May, 2013.
- Registration items you can modify online are limited to personal information (except e-mail address).
- Name changes will be accepted up until 15 June, 2013, and will incur an administrative fee of USD 80.00.
- The passport should be presented to pick up the registration pack at the conference.

REGISTRATION FEES

Please note the reduced fee is reserved for delegates from low income countries, patients and students.

1* Low income countries: Please submit a copy of the delegate’s passport from a low income country to verify nationality by one of the following methods: (1) scan and upload a copy when you register online, or (2) send a copy by e-mail or fax to the secretariat. You also should present your passport when you pick up your registration pack at the conference.

2* Patients: For registration as a patient or patient representative, please contact the secretariat of 10th HTAi Seoul 2013 by e-mail (reg@htai2013.org).

3* Students: Please submit a letter written on official letterhead and signed by a department head or mentor to verify your student status by one of the following methods: (1) scan and upload a copy when you register online, or (2) send a copy by e-mail or fax to the secretariat. You also should present a copy of your student ID when you pick up your registration pack at the conference.

4* Workshop: The number of registrants for workshops is limited. In case the minimum number of participants (20 people) is not met, course(s) can be cancelled.

REGISTRATION COVERS

Payment can be made either through credit card or bank transfer. Any other payment method such as postal orders and travelers cheques will NOT be accepted.

PAYMENT METHOD

Credit card
- Paying with a credit card using VISA, Master, and JCB is available via online registration.
- The actual debit amount is subject to change according to the exchange rate.

Bank transfer
All bank charges for remittance must be borne by the registrants and should not be deducted from the total amount remitted.  Participants are required to pay the difference on-site.
- A copy of the bank remittance receipt should be faxed or emailed to the secretariat.
- All bank transactions must be clearly marked with the registrant’s name.

CANCELLATION POLICY

- All Cancellation requests must be made in writing to the secretariat via e-mail or fax.
- All bank charges for remittance will be deducted from the amount of the refunds.
- Name changes will be accepted up until 15 June, 2013, and will incur an administrative fee of USD 80.00.
- All approved refunds will be processed within 4 to 6 weeks after the close of the conference.
